Documentation Guidelines听

Students who听wish to be considered for reasonable accommodations must submit current documentation (i.e., medical/educational/diagnostic reports) from听licensed physicians, psychologists, or other qualified professionals as well as supporting historical records and materials.听 School plans such as IEPs or 504 Plans are helpful, but do not substitute for complete and current documentation.听

Documentation guidelines are provided in the interest of assuring that documentation is complete and听supports requested accommodations, academic adjustments, and/or auxiliary aids as being necessary.听 A diagnosis of a disorder/impairment alone does not qualify an individual for an accommodation under听applicable federal laws.

For the purpose of receiving consideration for reasonable accommodations at the University of Louisiana at Monroe, an individual must have an impairment that substantially limits one or more major life activities in comparison with the average person in the general population.听

It is the responsibility of the student to obtain documentation and present a copy to the Self-Development, Counseling, and Special Accommodations Center. When requested, it is also the student鈥檚 responsibility to obtain additional records and/or evaluation/testing data.

Please be aware that the University of Louisiana at Monroe reserves the right to determine if a condition or diagnosis rises to the level of a disability under applicable federal laws and听decides what constitutes necessary accommodations according to the documented nature of the disability as well as the fundamental nature of the program.听
